to Receive Your Rebate:
1.
Get a contact lens fitting and eye exam!
(Purchase must be made within 90 days
of the date on your eye exam receipt in order
to qualify for rebate.)
2.
Purchase the required quantity of AIR

contacts as indicated above.
3.
Complete
the Official Mail-In Rebate Certificate we
will send you with your contact lens order.
(May arrive in a separate mailing.)
4.
Attach your original dated sales receipt
for the qualifying purchases (photocopies
are not acceptable), and a copy of your valid
eye exam and contact lens fitting receipt.
5.
Mail in the package UPC codes as indicated
on the certificate, your original
sales receipt indicating total purchase quantity,
a copy of your original eye exam and contact
lens fitting receipt and your Official Mail-In
Rebate Certificate (mailed to you from ExpressContacts.com)
to the address on the certificate.

If you are personally filing a claim for reimbursement
