Please complete all
5 steps to receive your rebate check
1. Get
an eye exam from an Eye Care Professional
(purchase must be made within 90 days
of the date on your eye exam receipt in order
to qualify for this rebate).
2.
Purchase the required number
of boxes of Bausch & Lomb®
Brand Contact Lenses from January 1, 2008
- December 31, 2008. Circle the eligible purchase
on your receipt or invoice (refer to the list
of products on this page).
3. Visit www.bausch.com/rebates
and input the single-use PERSONAL CODE we
send you to complete the offer form online.
Limit 1 rebate per personal code. Complete
all information requested online accurately.
 4.
Remove the original UPC codes
from the SofLens boxes. For
the $50 rebate, remove 8 UPC codes from the
SofLens® Toric, SofLens® Multi-Focal, or SofLens®
daily disposable boxes (SofLens®38 is not
eligible for the 8 box $50 offer). For the
$30 rebate, remove 4 UPC codes from the SofLens®
Toric, SofLens® Multi-Focal, or SofLens® daily
disposables boxes or 8 UPC codes from
the SofLens®38 boxes. See example
to the right. Copies of the UPC do not qualify.
5. Mail a copy of your
receipts(s) for eye exam and product
purchase, original UPC codes, and completed
offer form to the address provided
on the offer form. Submissions must be postmarked
within 60 days of purchase.
Please
allow 6-8 weeks for delivery of your rebate
check.
IMPORTANT:
Photocopy your entire submission for your
records. You could be required to mail, e-mail
or fax these photocopies.
Rebate
checks will be mailed to qualifying customers
within 60 days of the postmark date on your
mailed request. If you have questions about

Your right to receive this rebate will not
be earned unless you satisfy each of the Conditions
of Acceptance described above. Your failure
to follow each of these steps is a rejection
of this rebate offer. Resolution of any disputes
will be governed by New York law. The Mail-In
Rebate Offer is valid only for purchases of
Bausch & Lomb SofLens®38
contact lenses, SofLens®
Toric lenses, SofLens®
Multi-Focal lenses, or SofLens®
daily disposables ("qualifying products").
Purchase of the qualifying products is required
to qualify for this rebate offer; no product
substitutions, deletions, or additions are
allowed, regardless of information learned
from other sources. This offer cannot be combined
with any other offer unless specified in writing
by Bausch & Lomb. This offer is available
to all customers with mailing addresses in
the US and Puerto Rico. Purchases made in
or for delivery to other countries are not
eligible. All rebate checks will be paid in
U.S. dollars. Bausch &
Lomb is not responsible for lost, late, damaged,
illegible, misdirected or postage-due submissions.
Your rights to this offer cannot be assigned
or transferred and this offer is void where
taxed, restricted or prohibited by law. All
submitted materials become Bausch & Lomb
property and will not be returned. One rebate
per person per 12 month period. Limit five
(5) rebates per address per 12 month period
except where prohibited. Excessive submissions
contititute fraud and may result in federal
prosecution under the U.S. mail fraud statutes
(Title 18, USC 1341 and 1342). NOTICE
TO CONSUMERS: If you are personally
filing a claim for reimbursement from a third-party
